The Health Care & Benefits Division (HCBD) is a division within the Department of Administration. HCBD manages the State of Montana Benefit Plan (State Plan) which provides benefits (i.e. medical, prescription, dental, vision, life) for State of Montana employees, retirees, legislators, and survivors.

If you were eligible for State Plan benefits during the 2025 calendar year, you may request a paper copy of your IRS Form 1095-C.

Active employees and retirees may login to the benefits enrollment system to download a digital copy of their IRS Form 1095-C. Terminated employees and retirees must request a paper copy.

EOI is also known as "evidence of good health." It is the process that determines if a person is healthy enough to be considered eligible for the amount of insurance coverage they are requesting. Depending upon when you enroll for coverage, or the amount of coverage you are requesting, you may be required to complete EOI prior to coverage becoming effective.

State Plan Vendor Data Breach FAQs


Conduent Data Breach (BlueCross BlueShield of Montana)

BCBSMT is the State Plan's Medical Plan third party administrator and currently processes medical claims for the State Plan. If you or your dependents were impacted, you will receive a letter in the mail from Conduent.


Healthcare Interactive Data Breach (Allegiance)

Allegiance was the State Plan's Medical Benefit third party administrator from 2016 - 2022. Allegiance used to process medical claims for the State Plan. If you or your dependents were impacted, you will receive a letter in the mail from HCIactive.
